Planning Your Visit

Prior to your arrival, we will complete as much pre-operative work as possible.  You can take the first step in that process by completing the new patient form and medical history questionnaire out online and submitting prior to your appointment.  Another option is to print out the forms, fill them out and return them on the day of your appointment.  You will also want to consult with your primary care physician before coming to Gainesville so that any labs or necessary tests for medical clearance can be obtained.  Bring any medical documentation and a list of medicine with you for your appointment.  A friend or family member must come along so you will have someone to take care of your needs during recovery.
